Contacting Us 

We encourage you to call the office if you have any problems or questions. We ask that you let the receptionist know the nature of your call, our receptionists do not have medical backgrounds, they will notify Dr. Sullivan and he or a nurse will return your call. Dr. Sullivan can be reached directly for non-emergency, non-health related questions, concerns, or comments at his email address below.

Contacting us After-hours  

IN CASE OF EMERGENCY  CALL 911 

For emergencies call 911 or go to the Emergency Room or Labor and Delivery.  For non-emergencies you can reach our answering service by calling our office.  Dr. Sullivan or an on-call physician or nurse will be notified to return your call. If your call is not returned in thirty minutes, please call again. If your phone has caller ID, make sure it is set to accept blocked calls when expecting our call.

Prescription Refills 

INFORMATION FOR ESTABLISHED PATIENTS ONLY

Option 1: Request prescription refills by calling you pharmacy two or more business days before your medication runs out. The pharmacy will send a request to our office for authorization. Please plan ahead and note our office is closed on Fridays. 

Option 2: Call our office and give the name and dosage of your medication and your preferred pharmacy.  Although we try to handle all requests on the same day, please allow two business days to ensure you will not run out of your medication.

Lab Test Results

We are equipped with an on-site laboratory for your convenience. The lab is owned and operated by {Lab Company?}, all billing issues should be directed to {Lab Company?}.  The amount of time it takes to receive results is dependent on the lab.  Results can take anywhere from a few days to a month. On average pap smear results take 2 weeks. Patients will be notified by telephone of their results if they are abnormal. The lab results are available to view online by setting up an account with our patient portal. If you desire lab results, login to the patient portal or call our office during business hours: Monday-Thursday, 8:30am - 6pm.  Our office is closed on Fridays, but we are available to take your calls from 8:30am - 3pm

What to bring to Office Visits 

Please bring a Photo ID and all current health insurance cards with you to your appointment. Patients are responsible for paying co-payments at the time of their visit. We take cash and credit cards. If you have questions regarding our fees, you are welcome to call our office, we are happy to answer any and all of your questions.

Insurance and Billing 

We bill both primary and secondary insurances. Dr. Sullivan is not currently accepting Medicaid or Medicare patients with the exception of Medicaid OB patients. All patients are responsible for paying co-payments at the time of their visit. If you have questions regarding insurance, fees, or billing please call our office and ask for the billing department.
